Cog Hill Golf & Country Club When: Monday, September 8, 2008 Time:
1:00 pm Shotgun Start, Scramble Format Cost: $195 includes green fees and cart, a box lunch, and the 19th Hole Beer Bash. Transportation: Round trip transportation will be provided. The bus will leave from the Marriott Oak Brook Hills Resort no later than 12:00 NOON and return to the Hotel by approximately 7:30 pm. Driving Directions: If you choose to drive yourself to the Golf Course, please arrive no later than 12:30. Click here for driving directions. Golf Club Check-in: When you check into the Marriott Oak Brook Hills Resort, you can check your clubs at the Bell Stand and your clubs will be delivered to the golf course. If you are not staying at the Oak Brook, but would like your clubs delivered to the course, please check them at the Bell Stand Monday morning, September 8th, by 9:00 am. Club Rental: If you need to rent golf clubs, call Pro Shop (866-COGHILL x318) and tell them you are golfing in the LDC Forum Tournament on Monday, September 10th and if you need men's/women's and right/left handed clubs. Please provide a credit card to secure your rental and speed up the process on site. Foursomes: You will receive your foursome assignment when you check into the conference on Monday. Requests for foursomes will be considered if received prior to August 15. You will have an opportunity to put in a foursome request when you complete your registration. 19th Hole Beer Bash: Plan on attending the 19th Hole Beer Bash from 6:00 - 7:30 pm. Non-golfers are also encouraged to attend. Cancellation Policy: Golf cancellations must be received by August 15th to be considered for a refund. Substitutions are allowed anytime. Requests for foursomes will be considered if received prior to August 15th.
